To ensure a smooth enrollment process, please submit all required documents by the Friday before your child’s first day of care. All forms must be completed and signed to finalize your child’s enrollment at Eeny Meeny Preschool.

- Transportation Consent Form
- Completed Medical Examination Form
- Immunization Card / Records
- Non-Medication Consent Form for Diaper Cream (Vaseline, A&D, or Aquaphor)
- Individual Health Care Plan for a child with special health care needs (if applicable)
- Individual Allergy & Anaphylaxis Emergency Plan (if applicable)
- Parent Handbook-Contract (Initial each page and sign the last page)
- Contractual Childcare Agreement
- Public Assistance Confirmation Receipt (for HRA subsidy recipients)
- CACFP Form
- Infant Feeding Arrangement Form (for infants enrolled)
- COVID-19 (SARS-CoV-2) Attestation Form: One-time form required for all families
💵 Applicable Fees
- Registration Fee: $200 per child (non-refundable)
- Tuition Payment: First payment due prior to the first day of attendance (non-refundable)
- Security Enrollment Fee: Refundable
